Genu valgum deformity correction by distal femur translation osteotomy and anterograde intramedullary nailing

Background: Corrective ostetomy for deformity correction in adult patient with genu valgum has been performed past but associated many complications like wound complications, hardware irritation, delayed union long period of immobilization. Here we use intramedullary nailing along osteotomy to reduce these complications.Material and Method: 22 (7 females, 15 males) moderate severe (Intermalleolar distance more than 10 cm) age > 12 years who presented the OPD department, where epiphysial stapling is unpredictable, were selected. 4 patients had bilateral disease. 2 metabolic disorder. Rest all idiopathic valgum. Results: osteotomy, mean 20.6 which 7 female 15male over femur interlocking nail. Patient discharged after post-operative day advised physiotherapy. follow up 1 month, 3month, 6month, year years. All osteotomies united within 3 months except showed 4month duration. There no cases compartment syndrome, neurovascular injury, limb length discrepancy, implant failure, malunion. Full range motion achieved post operatively at hip knee joint. Pre op Post difference intermalleolar distance, tibiofemoral angle, lateral distal femoral angle mechanical axis deviation was found be statistically significant.Conclusion: We conclude that translation being closest apex i.e. The knee, a biomechanically sound leads complete axial realignment excellent apposition fragments without alteration.
